What do you need to have an IMSS Coverture?

 You need  the following:

 

1.    FM3 or FM2 and a copy of main pages.

2.    Passport and copy of main pages

3.    2 Front Color Picture (Passport Size)

4.    Proof of residency (Electric or Telephone Bill)

5.    Annual Fee.

6.    Name of Parents.

7. Birth Certificate

Note: If applying for Chapala: Marriage Certificate (translation), Birth Cerficate (translation) and Certified Power of attorney is needed.

20-39 years old                      1478.60

 

40-59 years old                      2210.10

Procurement Fee

 

New                     600.00

 

Renewal              500.00

**The requirements, quality and service varies based on the offices internal rules. Some offices are asking a Birth Certificate and  marriage certificate and a translation of  them.

RENEWING YOUR IMSS 

1. Bring the last year payment and the reference attached.

2. One front color Picture, Passport Size.

** Some IMSS offices request everything again.

WARNING: 

Make sure you keep the original copies of your payment  and application forms, they might be needed for future references.  Make sure to confirm it was indeed renewed!! .You can contact your local Office: Chapala Office 376-7653031. Jocotepec Office: 33-31375610. If late one day.... Start all over!

 

NOTE: The IMSS reserves the right to reject any application or person. If the person is lying at the time of filling the form and then is discovered later, it loses what it paid and must pay for services received in full. There is a period of 2 years of waiting for major emergencies.
